How to schedule an appointment via Direct Health
Wellness visits anytime, anyplace.
Jensen Family Wellness offers care through the Direct Health App (formerly Medici), a cutting-edge telehealth platform allowing for audio/video appointments and 24/7 messaging. It is HIPAA-compliant, offering a private and secure delivery of your virtual healthcare, including prescriptions and lab orders. Dr. Jensen is licensed in ALL 50 States.
Dr. Jensen’s rates begin at $117 for a single-person consult and $197 for more than one person (family rate). The consult rate may increase depending on the complexity of your needs.
NOTE: When you connect with Dr. J via the Direct Health App, a temporary hold of $117 may be placed on your card for up to 48 hours. You will be responsible for any insurance copay or deductible payment that is due upon completion of your text appointment in the app. Keep in mind that some (copay) or all of the cost of your visit may be covered by your insurance (see step 4 below for details). Otherwise, the payment card connected to your Direct Health account will be charged.
Step-By-Step Guide
for new patients:
1. Create an Account on the Direct Health App for Patients
You will be asked to enter your full name, DOB, sex (per your birth certificate), and contact info. We recommend you OPT IN to text notifications to receive communication from Dr. Jensen.
2. Enter the code JENSQUE832
The invite code JENSQUE832 allows you to enter the app and access Dr. Jensen’s profile.
3. Add Preferred Pharmacy & Payment Method
TO ADD YOUR PHARMACY: Tap your Profile icon in the bottom right corner. Tap View Profile and scroll down to add your pharmacy. (Skip if instructed by a staff member.)
TO ADD A PAYMENT METHOD (required even if you plan to pay via insurance): Tap your Profile icon in the bottom right corner. Tap Payment, then Add a New Card.
(NOTE: Your card will only be charged if you will not be using health insurance. If you plan to use insurance, follow the next step.)
4. Send Us Your Insurance Card (if applicable)
Please complete either of the following methods to send us your insurance card:
OPTION 1: Email pictures/photocopies of your health insurance card, front & back, to our office manager Josie at [email protected].
OPTION 2: Message pictures/photocopies of your health insurance card, front & back, as an attachment in the Direct Health app.
We accept most PPO health insurance plans (Aetna, Cigna, BCBS, UHC) as well as Medicare and some AHCCCS plans. We do not accept any HMO plans at this time. If an Arizona resident, we accept MercyCare and HealthChoice if we are assigned as your PCP. We do not currently accept Humana.
5. Message Dr. Scott Jensen the Reason for your Visit
Find his profile by searching his last name JENSEN and city PINETOP-LAKESIDE, ARIZONA. Once in his profile, tap Message in the bottom right corner.
YOUR MESSAGE MUST INCLUDE THE FOLLOWING INFORMATION:
Reason for visit
Body weight & height of patient
2-year medical history (including past treatments/diagnoses with a doctor or hospital)
Allergies, chronic illnesses, current conditions, symptoms, etc.
Current medications and/or supplements
Feel free to explain any new issues, follow-up issues, or general questions. Be sure to tap the send arrow. You can expect further replies regarding your care through the Direct Health app.
