Dr. Jensen is licensed in all 50 States.

How do I become a patient?

To become a patient, download the Medici app, a cutting-edge telehealth platform. It is HIPAA-compliant and allows for both audio/video appointments and 24/7 messaging. Dr. Jensen has moved to serving patients primarily through this telemedicine platform. You can message him directly for appointments through the app. Dr. Jensen can prescribe supplements/medicine and order labs for you remotely.

New patients must create a Medici account (see next question for more detailed instructions) and send Dr. Jensen an initial message including the following important information:

reason for visit

bodyweight

– medical history (past treatments/diagnoses in either a hospital or doctor’s office)

allergies

– current conditions/illnesses

current medications/supplements

COVID test results and symptoms (if applicable)

How do I get the Direct Health App and contact the doctor?

The Direct Health App can be found on the App Store or on Google Play, available for download on any smartphone. Be sure to download the app that says “Text Your Doctors” by the icon, not the one that says “Doc.”

1. Create an account by providing the following:

– COUNTRY

– MOBILE NUMBER

– PASSWORD (8+ characters, 1 number, 1+ uppercase)

– FULL NAME

– DATE OF BIRTH – GENDER 

2. Agree to the terms and then OPT IN to text notifications so you can receive communication from Dr. Jensen. Enter the activation code JENSQUE832 to find us.

3. Finish your profile. Tap “PROFILE” in the bottom right. Tap “ADD PHARMACY” (skip if instructed to do so by staff member). Tap “PAYMENT” to arrange your preferred payment method. Tap “ADD NEW CARD” and fill out credit card info (even if you plan to pay with insurance). Tap “ADD,” and then tap “HOME” in the lower left and you should now be able to find Dr. Jensen’s profile on the home screen. 

4. Find and message Dr. Jensen. TO FIND DR. SCOTT M JENSEN’S PROFILE YOU NEED TO USE HIS FULL ADDRESS: 6682 LOWER RIDGE DR, LAKESIDE, AZ 85929 (FOR THE CITY YOU WILL NEED TO CHOOSE EITHER LAKESIDE, AZ OR PINETOP/LAKESIDE, AZ)

Once you have found his profile, TAP “MESSAGE” (under Dr. Jensen’s photo.) TYPE A MESSAGE TO DR. JENSEN AND INCLUDE THE FOLLOWING:

– Body weight of each participant in household

– Any treatments each participant has received in a HOSPITAL OR DR. OFFICE IN LAST 2 YEARS

– Chronic diseases (if any)

– Allergies (if any)

– Current medications and supplements each participant is taking

– If you have COVID, the date you tested positive and the symptoms you are having

Be sure to TAP THE ARROW AT THE BOTTOM OF THE PAGE TO SEND MESSAGE CONTENT TO DR. JENSEN. You can then expect a reply from him and have provided him with all the information he needs in order to take you as a new patient.

Can I pay with insurance?

We now accept most insurances and traditional medicare except Humana. These are mostly PPO health insurance plans (Aetna, Cigna, Blue Cross Blue Shield, UnitedHealthcare, etc.). We also take MercyCare in AZ.

If you would like to pay for your Medici visit with insurance, send a picture/photocopy of your health insurance card to max.jfm@gmail.com after you have joined Medici and contacted Dr. Jensen through the app.

What are Dr. Jensen's rates?

Dr. Jensen’s rates begin at $117 for a single-person consult and $197 for more than one person (family rate). However, the consult rate may vary depending on the complexity of you or your family’s needs. You can pay via credit card through the Direct Health App or through your health insurance.

How do I receive my prescriptions?

Dr. Jensen will send in your prescription to a pharmacy of your choice, usually the one indicated on your Medici profile, unless Dr. Jensen explains otherwise (in the case of certain COVID treatment prescriptions, for example).

The pharmacy will contact you for payment and arrange for local pickup when possible (for non-COVID treatment prescriptions) or arrange to Fed-Ex ship your medication to you (overnight delivery is available at Fed-Ex rates).

The best way to have a prescription refilled is to contact your pharmacy directly. Do not contact Jensen Family Wellness with questions about prescription pickup, shipping, supply, pricing, and refilling. Contact your pharmacy directly. .

Can I see Dr. Jensen in person or only online?

How do I get treatment for COVID?

Patients seeking care for acute COVID symptoms, preventative/prophylactic treatment, vaccine-related symptoms, or long-term COVID symptoms, follow the steps to become a new patient and then include a description of your case (symptoms, severity, test results, and other relevant details) in your message to Dr. Jensen.

Depending on whether a COVID-19 patient requires immediate intervention, Dr. Jensen will do his best to expedite their treatment and prescription orders so they can be treated as soon as possible.

What does Dr. Jensen believe about COVID treatment/vaccines?

It is Dr. Jensen’s priority to stay up-to-date on research regarding COVID treatments, including vaccination, and he would like to make clear that his professional opinion is always subject to change as new information is made available. In congruence with recent research he recognizes the validity of natural immunity to SARS-COV2.  

Dr. Jensen uses the I-CARE and I-RECOVER approaches from the FLCCC (Front Line COVID-19 Critical Care Alliance), which entail both preventative/prophylactic care and early outpatient protocols for COVID patients, as well as treatment of long-term symptoms.

Dr. Jensen also treats patients who have received the vaccine and are experiencing adverse effects or sensitivities as well as patients who desire to proactively optimize their health.

He highly values individual freedom in making personal health decisions.

What if I do not hear back from Dr. Jensen?

Depending on patient volume at a given time, it may take between 24-48 hours to receive a response from Dr. Jensen through the Direct Health App. Be sure to OPT IN to text notifications or TURN ON push notifications in the Direct Health app, and check your inbox in case you have missed a message from Dr. Jensen. If you choose to call/text or leave a message through the website or email for our office manager Max (928-224-4270  max.jfm@gmail.com), he will do his best to respond to your questions and concerns within 3-5 business days.
